Do I need a referral from my physician?

Some insurance plans require a referral from your primary physician. Please contact your insurance provider to verify coverage and determine if you need a referral.

How should I prepare for my first visit?

To make your appointment more efficient, please bring:

  • Insurance card(s)
  • Driver’s license
  • Referral, if applicable
  • Competed paperwork, mailed prior to your scheduled appointment for convenience
  • All information pertaining to the reason for your visit, such as:
    • Consultation summary
    • Imaging reports including previous ultrasound testing, MRI scans, CT scans, & X-Rays
    • Medical/Surgical history
    • Medication list
