Some insurance plans require a referral from your primary physician. Please contact your insurance provider to verify coverage and determine if you need a referral.
How should I prepare for my first visit?
To make your appointment more efficient, please bring:
- Insurance card(s)
- Driver’s license
- Referral, if applicable
- Competed paperwork, mailed prior to your scheduled appointment for convenience
- All information pertaining to the reason for your visit, such as:
- Consultation summary
- Imaging reports including previous ultrasound testing, MRI scans, CT scans, & X-Rays
- Medical/Surgical history
- Medication list
